POLICY STATEMENT
Any equipment loaned from Campus Health Services for patient use will be tracked to prevent loss. This policy specifically refers to the loan of wheelchairs.
REASON FOR THIS POLICY
To identify a mechanism for tracking any equipment loaned to patients in Campus Health Services (CHS).

DEFINITIONS

POLICY
1. A log will be maintained in the clinic for tracking purposes. This will be monitored daily by a designated person.
2. Should a patient request/need a wheelchair, a Loaned Wheelchair Agreement form will be signed and the following information will be noted:
a. Name
b. Student ID number
c. Address
d. Phone number
e. Wheelchair ID number
f. Patient’s signature
g. Date
3. Wheelchairs are loaned for no more than 24 hours or at the discretion a Registered Nurse (RN).
4. Wheelchairs may be loaned to a student’s family member or visitor for emergent needs at the discretion of a RN.
5. Upon return, the agreement form will be updated with a return date and the signature of the CHS employee receiving the wheelchair. A copy of the completed form will be scanned into the patient’s medical record.
6. If a wheelchair has not been returned within the specified timeframe, an attempt will be made to notify the patient/student by phone, text, or secure email message and documented on the agreement form. If after three attempts no contact is made, a penalty fee may be applied to the patient’s/student’s LOUIE account to cover the cost of the wheelchair.
7. The patient/student is responsible for any damages or irresponsible use of the wheelchair while it is in their custody.
