Policy
New employees of Campus Health Services/Medical Services should complete the following as soon as possible upon employment:
The CHS Administration initiates a new hire packet and the employee completes the packet on line. This packet includes all payroll and tax information. The new employee must take two of his/her original social security card/drivers’ license/passport to CHS Administration and the staff completes the on-line I9 form for the employee.
With assistance of supervisor, and as soon as is possible, schedule attendance at the university’s new staff orientation, where the employee will complete forms and receive instructions and information concerning employment benefits and university personnel policies.
Report to Parking Services in order to register a vehicle, and obtain a parking permit.
Obtain proof of freedom from active Tuberculosis at time of employment and on a yearly basis.
Review NAU’s personnel policy and procedure manual (available online at nau.edu/hr).
Review the Campus Health Services policy and procedure manual (in NAUshares)
Receive orientation to the health center from the appropriate supervisor.
Receive HIPAA training, take the Privacy and Security Quiz and print a copy of the training documentation for the personnel file.
Complete the following on-line trainings:

Preventing Workplace Harassment Training
Information Security Essentials
CERT Conduct, Ethics, Reporting and Transparency Disclosure
Clinical Staff
If not previously vaccinated w/the Hepatitis B vaccine, arrange to receive the Hep B vaccine if contact with blood is involved in work duties or complete a HBV information form (found in policies & procedures, CLINIC13).
Take the OSHA-regulated blood borne pathogen training online by contacting the Director of Nursing at Campus Health Services
Meet with appropriate supervisor, Medical Director, Nursing Supervisor, Chief Medical Technologist, and IT Lead to:
Assign/approve privileges (for provider staff)
Provide current verification of professional licensure and DEA (if applicable)
Provide verification of current certification in CPR. Current certification and proficiency must be maintained while employed.
